The website with forms that I have choosen is Shells Fuel rewards. There was a lot of mostly errors. On the reset password page there were 640 Errors and 1368 warnings. On the first page there were 657 errors and 1184 warnings. I would rate design 10, because it layout nicely for anyone can use it to sign-up for there rewards program. For the usabiity, I would rate a 10, it is really users friendly. I definely it meets it purpose.
